Central Coordination-Placement Considerations
As a reminder, we are no longer requiring the electronic student profile for placement consideration. Please ask your staff to discard the link to prevent them from spending time on this when it is no longer necessary. Upon review of placement packets, the following must be included:
A District Service Request (DSR); please identify if you would like the packet to be submitted to an integrated and/or separate center program and the type of program you are interested in (i.e. EI, ASD, SXI, etc.).
The Placement Summary which was recently sent to you and is also available on the SEAOC Moodle.
The most recent IEP and any amendments, if applicable.
Current reports (educational, medical, therapeutic, etc.)
If there is a behavioral need, a Functional Behavioral Assessment and a Behavior Intervention Plan must be included.
Any other relevant information that you feel may assist center program staff in their consideration.
Following the 30-day timeline, or sooner if an IEP is held prior, please inform Central Coordination via secure dropbox of the outcome of the consideration. It is the resident district’s responsibility to do this.
Remember: An IEP must be held for all students for which a placement packet has been submitted… even if the student remains in the district. All placement decisions must be made at an IEP.
